In the first of a two-part series on the subject of erectile dysfunction (ED),
Paul Cappelleri, a South African man who has been in the United States since 1994, was diagnosed with ED in 2000.
In 2003, he underwent a radical prostatectomy in South Africa.
The men had to be treated for prostate cancer, and in 2006 he went back to the US and began having sexual intercourse with a woman. He was prescribed a medication called Estrace, which treats a condition called benign prostatic hyperplasia (BPH).
He has not had any serious side effects and has never had any serious injury.
“I was not a candidate for this treatment. It was a mistake. I had never had a problem, but I had no reason to think that this was the issue that I was concerned about,” said the man, who has a history of heart disease.
He has a history of diabetes, and was diagnosed with diabetes in the US in 2005. It is not known how long it took for the medicine to work.
Cappelleri was given an experimental medication for ED in December 2006 and was started on his own.
“It has been my hope that this treatment will work for the man who has had one in his life, but it has not,” he said.
“He has a history of having high blood pressure and diabetes and it has had no effect. It has not, in my opinion, made a difference.”
He is now in South Africa and is taking another medication.
“I have had a lot of good luck with the treatment. I have no idea what it is that is helping. I have had no problems in the past. It is not a magic pill. It is a treatment that will work for you and for your life.”
The man is currently in the United States.
“He has a history of high blood pressure and diabetes and it has had no effect.
Cappelleri has not had any major injury.
“I have had very few problems and I have not had any injuries. The only thing I have known is that I have had to go to a hospital because I did not get an erection,” he said. “It was a shock.”
The man said he has never had a problem, but he has had no reason to think that this was the issue that he was concerned about.
“I had no reason to think that this was the issue that I was concerned about,” he said.
“I have been in the US since 1995, and it is not my fault, but it has not been my fault.”
In 2008, a man in South Africa was diagnosed with high blood pressure and was prescribed Estrace.
Cappelleri had his prostate removed and was prescribed a treatment called. The medication did not work, and he was diagnosed with ED and was prescribed Estrace.
“I had no problems at all and my erection lasted three or four hours. I felt fine, and I didn’t have any problems,” he said.
“I am currently taking two tablets of Cialis (tadalafil) and I am taking two pills of Estrace. It is not my fault. It is not my fault.”
Cappelleri was diagnosed with ED in 2002 and has been taking ED pills for several years. He has no other medical problems, and his wife has said he has never had a problem.
“I am not a person who is embarrassed about it, but I am embarrassed. I would never do anything to hurt myself,” he said.
“I would like to have that feeling again, but I am not going to do anything to hurt myself.”
The man has had no other injuries other than his prostate removed, and he is not taking any other medications.
“I am currently in the US.”
Cappelleri is currently taking two doses of tadalafil, and his symptoms are severe enough that he should be referred to a specialist, he said.
“I am not going to do anything,” he said.

1.Nitrates (e.g., nitroglycerin, isosorbide dinitrate): Cialis and nitrates both lower blood pressure. Taking them together can cause a significant and potentially life-threatening drop in blood pressure (hypotension), leading to dizziness, fainting, or even heart attack or stroke. It is contraindicated to use Cialis with nitrates or recreational "poppers" (amyl nitrites).
2.Alpha-blockers (e.g., prazosin, terazosin, tamsulosin): Both Cialis and alpha-blockers relax blood vessels, which can lead to excessive blood pressure reduction when used together. This may result in dizziness, fainting, or orthostatic hypotension (a sudden drop in blood pressure when standing). If needed, your doctor may adjust the dosage or recommend spacing the medications.
3.Other antihypertensive drugs (e.g., atenolol, propranolol, enalapril, lisinopril): Cialis can enhance the blood-pressure-lowering effects of these medications. While this interaction is usually less severe than with nitrates or alpha-blockers, it can still cause dizziness or fainting in sensitive individuals. Close monitoring of blood pressure is recommended.
Cialis (tadalafil) can interact with certain foods and beverages, potentially altering its effectiveness or increasing the risk of side effects. Here are the key drug-food interactions to be aware of:
If you take Cialis more than once, it is recommended that you stop taking it and seek medical attention. This is especially true if you experience serious side effects, such as angina, heart failure, or blood pressure changes.
Cialis has been shown to cause some mild side effects. Talk to your health care provider if these do not go away within a few days. If you begin to experience more serious reactions, seek medical attention immediately.
Common side effects reported from Cialis use include:
More severe side effects include:
If you experience any of these side effects, seek medical attention immediately. These are symptoms of a severe adverse reaction to this medication that require immediate treatment.
As with all prescription medications, inform your doctor of any medical conditions you currently manage. Tell them about any and all medications, prescription drugs, and supplements you are taking before starting treatment with Cialis. Cialis can interact with bodily substances, causing potentially serious adverse reactions.
Specifically, you should inform your health care provider of any alpha-blocker, antifungal, HIV protease inhibitor, or high blood pressure medication you are taking. Remember to mention any herbal products you use, especially St. John’s wort.
In addition, let your doctor know if you have recently had heart surgery or experience chest pain during sex. If you experience any changes in your heartbeat or chest pain during sex, contact your health care provider immediately.
